Off Schedule Appointment Request

Would like to make an appointment, but can't find a day or time on Laurie Jean Ellis' schedule that works for you? No worries! Laurie accepts several evening and weekend appointments each week. You may request one of the special time slots. Just follow the step-by-step instructions below:

Step-By-Step Guide for Requesting an      Evening/Weekend Appointment 

 

  FIRST - Decide which appointment type you need. Choose that appointment and pay for it.  

Review of your completed Health & Nutrition Questionnaire, Assess health symptoms and concerns, and collaboratively develop a Personalized Health & Nutrition Plan.

Health & Nutrition Counseling 60 Minute Session for the purpose of implementing your Personalized Health & Nutrition Plan. This may include self-management training, learning about what is going on inside your body, nutrition education, develop personalized nutrition plan, healthy lifestyle habits, learn skills and tools to support habit change, medication usage, nutritional supplements, and where to find more help.

Follow up on clients success with Personalized Health & Nutrition Plan [30 minute session]

SECOND - Laurie will be notified that you have paid for an appointment that is not been placed on the regular schedule. She will be expecting to receive an email from you. 

 

THIRD - Complete the form below, requesting an evening/weekend appointment and submit. Laurie will receive this form via email. You should receive an email response from Laurie within 24 hours after submitting the form.

FOURTH - Complete appropriate forms prior to your scheduled appointment. The forms and instructions about how to complete the forms are on the MAKE APPT page. 

FIFTH - Attend your virtual health & nutritional counseling appointment using the private Google Meet link sent to you.
