HOW CAN YOU BENEFIT FROM WORKING WITH A HEALTH & NUTRITION COUNSELOR?
Laurie Jean Ellis, is our primary Health & Nutrition Counselor. She provides health and nutrition education to individuals, to families, and to groups. Individual and family sessions may be in person or via video conferencing.
Laurie's scope of practice as a health and nutrition educator is to present information and training to promote skill development and knowledge that will help the client(s) adopt health supporting lifestyle habits and eating behaviors to optimize the client’s nutritional status, prevent disease, and maintain or improve the client’s health and well‐being. Health and nutrition information and training includes:
- Principles of good nutrition and food preparation;
- Food to be included in the normal daily diet;
- Essential nutrients needed by the body;
- Recommended amounts of the essential nutrients based on established standards;
- Actions of nutrients on the body;
- Effects of deficiencies or excesses of nutrients;
- Foods, nutrient supplements and dietary supplements that are good sources of essential nutrients;
- Lifestyle habits that support and damage health; and
- Behavioral change tools and skills to support the adoption of health supportive habits.

ARE YOU WONDERING WHAT TYPE OF APPOINTMENT TO SCHEDULE?
There are 3 types of appointments. Each has a time sequence and a purpose. There is paperwork that needs completed and submitted via email prior to a few appointment types. Here is what you need to know before choosing and scheduling an appointment with our Health & Nutrition Counselor:
Initial Consultation & Health Assessment
This is always the first appointment that a client should schedule. During this appointment the the Health & Nutrition Counselor will review of your completed Health & Nutrition Questionnaire, explore health symptoms, listen to concerns, and collaboratively develop a Personalized Health & Nutrition Plan.

Health & Nutrition Counseling
This is a 60-minute session for the purpose of implementing your Personalized Health & Nutrition Plan which was developed during your Initial Consult. This appointment may include self-management training, learning about what is going on inside your body, nutrition education, develop personalized nutrition plan, healthy lifestyle habits, learn skills and tools to support habit change, medication usage, nutritional supplements, and where to find more help.
There is a lot of work that is accomplished during this appointment type. Most clients require more than one Health & Nutrition Counseling Session.
Follow Up Health & Nutrition Session
The purpose of this appointment is to follow up on a client's success with their Personalized Health & Nutrition Plan. Many clients schedule several follow up appointments, spaced out over several weeks for the purpose of ongoing follow up and accountability with their Health & Nutrition Plan. This appointment type is also used to tie up loose ends and finalized the client and counselor working relationship.

Please read this Client Disclosure and Consent Form prior to scheduling your Initial Consultation & Health Assessment appointment.
Read it in its entirety! You will be asked to verify that you have read this consent, understand it, and agree with it, when you schedule your first appointment with Positive Patterns for Life.
Complete the forms to the right as follows:
1. Any new client WITHOUT diabetes, complete the Health Nutrition Questionnaire.
2. If you are making an appointment for diabetes, prediabetes, or gestational diabetes - complete both these forms to the right.
3. Any new client that wants help with eating healthier or weight loss, complete the Food and Activity Log.
