DIABETES SELF-MANAGEMENT TRAINING (DSMT) WILL HELP YOU LIVE HEALTHIER WITH DIABETES!

The program for Type 2 Diabetes includes 6 live telehealth classes and 2 on demand bonus sections. In addition to each class and bonus section, participants receive handouts, booklets, tracking logs, and educational bonus videos. Class content is as follows:
CLASS ONE
- Understand your body better (i.e. anatomy of diabetes explained, how it affects your body)
- Self-monitoring (i.e. how to test blood sugar, when to test, blood sugar goals)
- Taking medication (i.e. understand how medication works, when and how to take it)
CLASSES TWO & THREE
- Healthy eating (i.e. balanced diet, healthy choices, portion control, label reading, meal planning)
CLASS FOUR
- Active living (i.e. exercise safely, learn benefits, set physical activity goals)
CLASS FIVE
- Dealing with potential problems (i.e. high & low blood sugars, sick days, factors that affect blood sugar)
CLASS SIX
- Reducing risk of complications (i.e. checkups, lab tests, adopt health habits that decrease risk for kidney - vision - cardiovascular complications)
ON DEMAND BONUS SECTIONS

Gestational Diabetes Program Description
The program includes 2 one-on-one telehealth sessions plus a total of 9 educational videos (listed below). Pregnant participants will find multiple informative handouts, PDF booklets, and logs (blood sugar, food, activity). The following topics will be covered during this program:
- Understand your body better (i.e. anatomy of gestational diabetes explained, how it affects you and your unborn baby)
- Self-monitoring (i.e. how to test blood sugar, when to test, blood sugar goals)
- Healthy eating (i.e. balanced diet, healthy choices, portion control, label reading, meal planning, food safety during pregnancy)
- Active living (i.e. benefits of physical activity, safety precautions to follow during pregnancy)
- Habit change training and skills (i.e. write successful goals, positive focus, develop craving management plan)
- Easing stress (i.e. coping with diabetes, managing stress, build resilience, combat negative thinking)

Program to be Released August 2026
Program for Youth with Prediabetes or Type 2 Diabetes
Positive Patterns for Life has a program specifically designed to help teens and preteens adopt healthier eating and lifestyle habits. The program was initially developed to help youth with high BMIs achieve a healthier weight. However, it was soon discovered that the program helped lower high blood sugars, reverse prediabetes, and bring blood sugars under control for youth who had type 2 diabetes. Pediatric clinics who initially began using Back in Whack for Teens (BIW4TEENS) for a weight management program, quickly realized this program was an effective treatment intervention for prediabetes and type 2 diabetes in youth.
