BiW4Adults Program Commitment Contract
This contract serves two functions. First, it help clients understand what is expected of them as they work through the program. Second, it helps client's make a solid commitment with the clinic to do their best to show up for clinic appointments and to stick with the program for a year.
BiW4Adults Program - Multi-Client Log
The multi-client log makes it easy to keep track of all the clients that are working through the BiW4Adults program and how far each client has worked through program materials. If you are wondering which clients have missed monthly clinic appointments, you don't have to rifle through a bunch a client charts, just grab this log. In one glance you will know who is up to date on clinic visits and who needs a reminder to make their next clinic appointment.
BiW4Adults - Client Progress Log
This is a very useful log that helps you track program progress for individual clients. This log also includes all the Funky Frases for each section. Funky Frases are built into the program as a method of client accountability. Clients are given Funky Frases during video sessions. This form makes it convenient for staff - all they need to do is check the box by any given Funky Frase to verify that a client did know the frase for each session.
BiW4Adults - Clinic Visit Documentation Forms
There are two documentation forms here. One form is for the initial clinic visit and the second one is for all of the follow up clinic visits. These forms do more that provide a space to record client progress with the program. They help staff ask engaging questions during clinic visits. These engaging questions build rapport with clients and make clients feel like they have program partner. Engagement and rapport increase client motivation and commitment to the program.
